SERVICE AUTHORIZATION

By scheduling an appointment, authorizing services, tendering payment, or delivering possession of a vehicle or vehicle keys to Service on Wheels Wheelz, doing business as Full Blast Mobile Wheel Repair, and/or its employees or independent contractors, whichever occurs first, I acknowledge that I have read, understand, and agree to the following Service Authorization.

I hereby authorize Full Blast Mobile Wheel Repair to perform the agreed-upon services and understand that Full Blast, its employees, and/or independent contractors are not responsible for loss of or damage to any vehicle(s), or articles left inside, due to fire, theft, or any other cause beyond their control and not resulting from their direct action.

I grant Full Blast, its employees, and/or independent contractors permission to operate the vehicle(s) on public or private roads as necessary for testing, inspection, or repair purposes. An express mechanic’s lien is hereby acknowledged on any vehicle serviced, to secure payment for parts, labor, and related materials.

I further acknowledge that certain finishes and coatings require a proper curing period, and I agree to refrain from taking the vehicle(s) through car washes or using chemical cleaners on the treated areas for at least 30 days following service.

I understand that any pricing provided prior to service is an estimate based on available imagery, visible damage, and/or customer description. I acknowledge that unforeseen or hidden issues such as prior improper paint preparation or concealed damage may affect the final scope of work. By working with Full Blast, I agree to pay the full amount of the final invoice regardless of whether it is higher or lower than the original estimate.

In the event of non-payment, I agree to be responsible for all reasonable costs incurred by Service on Wheels Wheelz, doing business as Full Blast Mobile Wheel Repair, and/or its employees or independent contractors in enforcing payment obligations. Such costs may include, but are not limited to, attorneys’ fees, court costs, collection agency fees, and reasonable administrative and enforcement costs, including internal time expended in connection with billing, collection, lien enforcement, or legal coordination, whether incurred before or after legal action is initiated.

Additionally, I agree to respect Full Blast’s request for privacy during the repair process and will make a good faith effort to ensure that I, and any employees or representatives of my company, comply with this request.
